Office Nurse; RN

Vital Health is seeking a Registered Nurse to provide direct patient care in an outpatient behavioral health and primary care setting. This role focuses on patient assessments, coordination of care, scheduling, and timely documentation.

• Complete nursing intakes and assessments
• Perform vitals, screenings, and medication reconciliation
• Execute provider orders and monitor patient progress
• Coordinate care and schedule follow-ups
• Maintain an active caseload
• Ensure timely, accurate EMR documentation
• Support clinic efficiency and patient flow
• Meet productivity and patient engagement expectations

• Active RN license (Ohio)
• CPR certified
• Experience in outpatient, behavioral health, or primary care preferred
• Strong time management and documentation skills
