We are searching for an experienced Licensed Practical Nurse (LPN) for a full‑time position at our Toledo and Maumee facilities. The LPN will work under the direction of the Director of Nursing and Charge Nurses.
Shift: Full‑time night shift, 6 p.m.–6 a.m.

- 1+ years of experience as an LPN in a detox or similar treatment facility environment
- State LPN license in good standing
- Maintains current CPR certification
- Detox and psychiatric nursing experience required
- Experienced in administering medication orally, subcutaneously, and IM
- Ability to recognize signs and symptoms of withdrawal pain and critically dangerous medical emergencies and respond appropriately
- Critical thinking skills, decisive judgment and ability to work with minimal supervision
- Ability to respond physically with swiftness to a patient emergency
- Ability to respond calmly and timely during an emergency or potential emergency situation
- Familiarity with ASAM criteria preferred
- Familiarity with JCAHO Standards of Care for Behavioral Health
- Electronic Health Record experience
The Detox Nurse guides patients through their detox process per the physician’s orders, educates patients about their condition, and assists the physician and ARNP with accurate clinical reporting of patient conditions, status, and proper clinical actions. The Detox Nurse conducts nursing assessments at intake, performs necessary discharge nursing actions, and completes routine reports required for insurance patients.
Key responsibilities include:
- Administer prescribed medications as ordered by the medical team.
- Make independent decisions within the scope of the license involving patient care.
- Use sound judgment in informing the Chief Nursing Officer, Charge Nurse, and Medical Director of all decisions made and how they should be communicated.
- Monitor and record all aspects of patient care through the Electronic Health System, including nursing assessments at intake, shift assessments (CIWA and COWS), changes in patient condition and progress updates, discharge assessments and paperwork, adherence to universal infection control precautions, and reconciliation of medications at the end of each shift.
- Ensure proper handoff during shift change so the on‑duty nurse is fully informed, maintaining high standards of care for all patients.
