Position Overview
The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement.
Key Responsibilities- Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents’ physical, psychological and functional status, including implementation of Care Area Assessments (CAA) and triggers.
- Evaluate each resident’s condition and medical data to determine need for special assessment activities or amendments to the admission assessment.
- Prepare and electronically transmit timely reports to national Medicare and Medicaid databases.
- Develop written plan of care (preliminary and comprehensive) for each resident, identifying problems/needs and goals.
- Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
- Ensure MDS notes are informative and descriptive of services provided and resident response.
- Assist with care plan portion of residents’ discharge plan; evaluate and implement recommendations from committees regarding assessment and care plan functions.
- Current, valid state LPN or RN license.
- Three to five years’ experience in MDS or assessment role in senior residential care, healthcare, senior living or long‑term care.
- Current valid CPR certification.
Compensation will be determined based on the relevant license or certification held, and years of experience.
