The Clinical Support Supervisor is responsible for assisting with the oversight and coordination of clinical operations within the home health care agency. This position serves as a liaison between field clinicians, office staff, patients, physicians, and community partners to ensure the delivery of high-quality patient care while maintaining compliance with applicable regulations, agency policies, and professional standards.

Clinical Support & Staff Supervision
  • Provide day-to-day support to field clinicians, including RNs, LPNs, and HHAs
  • Assist with staff orientation, training, mentoring, and ongoing competency evaluations.
  • Monitor clinical performance and provide coaching to promote quality patient outcomes.
  • Participate in performance evaluations and corrective action processes as needed.
  • Serve as a resource for clinical staff regarding patient care concerns and regulatory requirements.
Patient Care Coordination
  • Collaborate with physicians, patients, families, and interdisciplinary team members to ensure continuity of care.
  • Review patient care plans for completeness, accuracy, and compliance.
  • Assist with case management activities and clinical problem-solving.
  • Ensure timely follow-up on physician orders, medication changes, and patient concerns.
  • Monitor patient satisfaction and address service-related issues promptly.
Quality Assurance & Compliance
  • Conduct chart audits and documentation reviews to ensure compliance with agency policies and regulatory standards.
  • Assist in preparation for state surveys, accreditation reviews, and agency audits.
  • Monitor clinical documentation for accuracy, timeliness, and completeness.
  • Participate in quality improvement initiatives and risk management activities.
  • Ensure adherence to HIPAA, infection control standards, and patient rights requirements.
Scheduling & Operational Support
  • Collaborate with scheduling staff to ensure appropriate clinician assignments and patient coverage.
  • Assist in managing staffing needs and resolving scheduling challenges.
  • Support the Director of Nursing in maintaining efficient clinical operations.
  • Participate in on-call rotation as assigned.
Communication & Leadership
  • Foster positive working relationships among clinical and administrative staff.
  • Facilitate effective communication between field staff and office personnel.
  • Participate in interdisciplinary team meetings and case conferences.
  • Promote a culture of professionalism, accountability, and patient-centered care.
Qualifications
  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the State of Ohio.
  • Minimum of two years of clinical experience, preferably in home health care.
  • Previous supervisory or leadership experience preferred.
  • Knowledge of home health regulations, Medicare guidelines, and clinical documentation requirements.
  • Strong organizational, communication, and problem-solving skills.
  • Proficiency with electronic medical records (EMR) systems and Microsoft Office applications.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
Physical Requirements
  • Ability to travel to patient homes and community locations as needed.
  • Ability to sit, stand, walk, bend, and lift 65 pounds (if needed).
  • Ability to use standard office equipment and electronic devices.
Work Environment

This position works in a combination of office and community-based settings and requires regular interaction with patients, families, healthcare providers, and agency staff.

Employment Status

Full-Time, Monday through Friday, 8am – 5pm.
