What does a Transitions at Home LPN do?Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours:
Full-Time
- 30-40 hours per week
- Monday through Friday 8:00 am - 4:30pm
- No Holidays
- Limited on-call rotation
- $28-$32 per hour starting wage, depending on experience

Qualifications /Job Requirements:
- Active Wisconsin LPN License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
