• Provide direct patient care and support in a long‑term care setting
• Administer medications following the 5 Rights and document accurately
• Complete scheduled resident assessments and monitor changes in condition
• Communicate significant changes in resident condition to physicians, families, and the care team
• Assist with CNAs’ tasks, respond to call lights, and support seamless daily operations
• Lead on the floor as needed and ensure thorough charting while reporting any concerns

• Active Wisconsin Registered Nursing license
• Two‑year nursing degree required; four‑year degree preferred
• Current CPR/BLS certification
• 1+ years of related nursing/LTC experience preferred
• Ability to work in a long‑term care setting
