The primary purpose of the Central Intake & Case Management Coordinator position is to facilitate the intake, referral, and admission process for assigned skilled nursing facilities while serving as a liaison between referral sources, hospitals, managed care organizations, residents, families, and facility teams.
Essential Duties and Responsibilities Referral Management Functions- Receive, review, and process referrals.
- Monitor referral activity for assigned facilities.
- Maintain referral tracking systems and databases.
- Ensure timely response to referral opportunities.
- Coordinate all aspects of intake from referral through admission.
- Review clinical, financial, and insurance information.
- Verify referral documentation requirements.
- Support timely admission decisions.
- Review referrals for appropriate level of care.
- Collaborate with facility clinical teams regarding admission suitability.
- Support continuity of care throughout transitions.
- Support transition-of-care initiatives.
- Communicate with families and responsible parties.
- Support readmission reduction initiatives.
- Assist facilities in achieving census goals.
- Monitor admission activity and conversion rates.
- Verify payer source information.
- Coordinate authorization requirements and approvals.
- Provide exceptional customer service.
- Maintain positive relationships with hospitals and community partners.
- Collaborate with admissions, clinical, business office and operations personnel.
- Promote teamwork and collaboration.
- Maintain referral tracking reports and admission metrics.
- Monitor referral conversion rates and outcomes.
- Ensure compliance with federal, state and company policies.
- Comply with HIPAA requirements.
- Protect resident confidentiality and privacy.
- Respect resident rights, dignity and personal choice.
- Associate's Degree required;
Bachelor's Degree preferred. - Healthcare admissions, case management, discharge planning or long-term care experience preferred.
- Understanding of Medicare, Medicaid and Managed Care processes.
