• Work collaboratively with a multidisciplinary care team to develop goals and plan interventions.
• Provide care and disease management coordination.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Conduct comprehensive clinical assessments including medical, behavioral, and social needs.
• Inform and work with patients and families regarding care coordination.
• Monitor patient compliance with care plans and perform reassessments.
• Serve as a liaison and advocate for patients and families.
• Assist in managing transitions of care across settings.
• Identify barriers, facilitate solutions, and connect to community resources.

• Education: Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred
• Experience: 3-5 years of nursing experience
• Licenses &
  
  Certifications:
  
  Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification required
• Specialty certification preferred
