The RN Care Coordinator for Orthopedics and Trauma at Cleveland Clinic's Hillcrest Hospital is responsible for collaborating with a multidisciplinary care team to manage high-risk patients' care effectively. • Work collaboratively with a multidisciplinary care team for high-risk patients.
• Provide care and disease management coordination.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Conduct comprehensive clinical assessments addressing various patient needs.
• Inform and work with patients and families regarding care coordination.
• Serve as a liaison and advocate for patients and families.
• Assist in managing transitions of care across settings.
• Identify barriers, facilitate solutions, and connect to community resources.

Education:

• Graduate from an accredited school of professional nursing.
• Bachelor of Science in Nursing (BSN) preferred.

Experience:

• 3-5 years of nursing experience.

Licenses &

Certifications:

• Current RN License in Ohio.
• Basic Life Support (BLS) Certification required.
• Specialty certification preferred.
