Licensed Practical Nurse (LPN) – Story Point

We are seeking an experienced licensed practical nurse to serve as a Nurse Supervisor within our assisted living and memory care facilities. In this role you will lead, manage, and train a team of caregivers while ensuring the highest standard of care for residents.

• Lead, manage and train your team of caregivers.
• Conduct evaluations of residents and report changes in condition to physicians.
• Utilize the Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR).
• Communicate effectively with team members, residents and families.
• Build strong relationships with residents, families and staff.

• Licensed Practical Nurse (LPN) in the state.
• Preferred:
  Assisted Living or Memory Care experience.
• Prior supervisory experience required.
• Strong punctuality, professionalism, and a passion for working with the elderly.
