- Assessment and Care Planning:
Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs. - Medication Administration:
Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects. - Wound Care:
Provide wound care management, including dressing changes, infection control, and monitoring healing progress. - Patient
Education:
Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self‑care and management of their health at home. - Monitoring and Evaluation:
Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes. - Collaboration:
Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient. - Documentation:
Maintain accurate and up‑to‑date medical records and documentation of nursing assessments, interventions, and patient responses. - Support and Advocacy:
Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.
- Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university.
- Currently licensed, in good standing, to practice in New York as a Registered Nurse.
- Minimum 1 year of community health nursing experience or equivalent professional experience.
- Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.
- Complies with accepted professional standards and practice.
