Licensed Practical Nurse (LPN) – Clinic – PRN

• Provide prompt communication and collaborate with patients' physicians.
• Work under a Nurse Practitioner or physician, providing direct patient care in accordance with scope and standards of practice.
• Assist in the assessment of patients by collecting data for analysis by an RN or physician.
• Perform plan of care interventions, including specimen collection and wound care.
• May administer medication.
• Document patient care given.
• Assist RN or physician with development of plan of care.
• Assist patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitor, document, and communicate patient status and reactions to interventions with RN, physician, patient, and families.
• Advocate for the physical and emotional well‑being of patients.
• Schedule appointments and procedures as ordered by the provider.
• Obtain insurance re‑authorizations as requested for testing and scheduled procedures.

• Current Kentucky or compact state LPN license.
• Graduate of an approved practical nursing program.
• Basic Life Support (BLS) certification, required within one month of hire.
• Associate’s degree preferred.
• Critical thinking skills.
• Decisive judgment and ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.
