- Assist
-DD adults in accessing medical, social, educational and other support services within and outside the agency. - Provide case management services to state general fund participants as assigned.
- Serve as the PASRR Coordinator and Case Manager.
- Ensure that all staff who provide PASRR services are trained.
- Oversee the provision of all PASRR specialized services.
- Complete and submit reports of services as required.
- Review all evaluations and recommendations.
- Meet with the individual to explain services and obtain verification that the services are desired.
- Develop Person Centered Plans / ISP’s / plans of care initially and annually for each person receiving services.
- Conduct monthly case manager visits to persons served.
- Review and approve direct service provider activities.
- Provide required number of home / nursing home visits per participant as outlined in the manual.
- Supervise direct support staff, including review and approval of services provided.
- Serve as the Michelle P Program Coordinator and Evaluator.
- Manage the MWMA portal, reviewing and completing all assigned services.
- Schedule, conduct, and submit Michelle P assessments and re-assessments.
- Provide support to Michelle P recipients, their families, and care providers.
- Assist with IDDD crisis services as needed.
- Attend staff meetings, in-service trainings and workshops as required.
- Ensure that all documentation of service provision is completed and submitted in a timely and accurate manner in accordance with regulations.
- Other duties as assigned.
- Bachelor’s degree in a human services field from an accredited college or university.
- Minimum of one year work experience with the IDDD population.
- Capable of visiting consumers in their own home, which may not be handicapped accessible.
- Capable of operating a motor vehicle.
- Must have a valid driver’s license and reliable transportation.
- Must pass annual criminal background and registry checks.
- Must pass urine drug screen.
