We’re seeking a Licensed Social Worker (LSW) to provide psychosocial support and care coordination in our Emergency Department at Loyola Medicine. This part‑time, 12‑hour night shift role requires an LSW to collaborate with a multidisciplinary team to assess, intervene, evaluate, and educate patients and families.

- Provide psychosocial assessment, intervention, evaluation, consultation, and education to patients and families as part of comprehensive care management services.
- Identify high‑risk patients from a psychosocial/financial perspective and provide information, support, counseling, care management, and referrals to appropriate resources.
- Serve as liaison for complex transitions/discharges between the hospital and community agencies to exchange clinical referral information.
- Contribute to training of social work professionals as part of the education program at LUHS.
- Coordinate details of placement and actual discharge of complex patients to appropriate agencies; discuss discharge plans and barriers with interdisciplinary team.
- Master’s degree in Social Work.
- Licensed Social Worker (LSW) required; LCSW preferred.
- Excellent communication skills.
- Ability to manage crisis situations calmly and effectively.
- Ability to work under stressful conditions and in difficult situations.
Salary Range: $30.00 – $41.59 per hour. Actual compensation may vary based on experience, qualifications, education, location, licensure, certification, and comparison to similar positions.
