Job Summary
The Case Manager is responsible for providing direct and indirect casework services to patients and their families and rehabilitation and skill development services
Essential Functions- Provides behavioral health case management services by linking, resourcing, advocating, and providing.
- Assists the team in identifying community and agency resources to meet patients’ needs, such as recovery programs, local sponsors, outreach programs for continued recovery.
- Links Patients to update information, assists with completion of online applications for employment, unemployment benefits etc.
- Advocates on behalf of patients in regards to unemployment, benefits, and services etc. (need ROI)
- Resources information for patients: i.e.; local food distribution times and locations, clothing closets, soup kitchen times and dates etc.
- Provides information and addresses, i.e.; potential jobs, recovery groups, essential faith-based family friendly events (i.e.; Easter hunt, fireworks display, holiday events and dinners.)
- Provide necessary forms to the Medical Director, properly and accurately completed, when the patient has special requests for take-out medications, dose changes, phase changes, MSW schedule, and medication changes.
- Ensures that patient confidentiality is protected by properly completing release of information to ALL outside referrals and contacts (i.e. medical coordination, information from other facilities).
- Completes discharge of a patient from program and is to be completed within fourteen (14) days from the date of discharge. The discharge form is to be completed in full and the file to be submitted to the Program Director.
- Contributes to the ongoing assessment and treatment planning process through participation in daily reports, case reviews, and treatment teams.
- Participates in a program of professional development including both external and facility required number of internal in-service programs and events.
- Work with the medical staff in securing urine samples for monthly urinalysis. Document results in the patient’s chart and in Methasoft program.
- Active involvement in Diversion Control through execution of random bottle recalls.
- Manages and prioritizes a caseload and provides behavioral health case management services to patients and their families.
- Ensures that necessary services are provided to individuals.
- Acts as the patient’s advocate and intermediary and ensures that providers meet the needs of the patients.
- Provides information to individuals/guardians/family members/advocates for decision-making related to service options to patients with appropriate releases.
- Solves problems and mediates with providers, agencies, and other team members.
- Other duties as assigned by Program Administrator.
