About the Role:

The Licensed Practical Nurse (LPN) plays a critical role in delivering compassionate and high‑quality care within assisted living environments. This position focuses on supporting residents' health and well‑being by providing routine nursing care, monitoring vital signs, administering medications, and assisting with daily living activities. The LPN collaborates closely with registered nurses, healthcare providers, and family members to develop and implement individualized care plans that promote safety and comfort.

Through vigilant observation and timely communication, the LPN helps identify changes in residents' conditions and ensures appropriate interventions are made. Ultimately, this role contributes significantly to enhancing the quality of life for residents while maintaining compliance with healthcare regulations and facility policies.

Minimum Qualifications:
  • Current and valid Licensed Practical Nurse (LPN) license in the state of employment within the United States.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated experience or training in assisted living care or a similar healthcare setting.
  • Strong understanding of medication administration and nursing care procedures.
Preferred Qualifications:
  • Experience working in assisted living or long‑term care facilities.
  • Additional certifications such as IV therapy or geriatric nursing.
  • Familiarity with electronic health record (EHR) systems.
  • Excellent communication and interpersonal skills to effectively interact with residents, families, and healthcare teams.
  • Ability to manage multiple priorities in a fast‑paced environment.
Responsibilities:
  • Provide direct nursing care to residents, including medication administration, wound care, and monitoring vital signs.
  • Assist residents with activities of daily living such as bathing, dressing, and mobility support.
  • Document resident care accurately and maintain up‑to‑date medical records in compliance with regulatory standards.
  • Collaborate with interdisciplinary teams to develop, implement, and update individualized care plans.
  • Observe and report any changes in residents’ physical or mental health to the supervising nurse or physician promptly.
  • Ensure a safe and clean environment by following infection control protocols and facility policies.
  • Educate residents and their families on health maintenance and disease prevention as appropriate.

The required skill of Assisted Living Care is essential for providing personalized and empathetic support to residents, ensuring their comfort and dignity throughout daily routines. Proficiency in medication administration and basic nursing procedures allows the LPN to deliver safe and effective care consistently. Strong observational skills enable the nurse to detect subtle changes in residents’ health status and respond appropriately. Preferred skills such as familiarity with electronic health records streamline documentation and improve communication among care teams.

Additionally, excellent interpersonal and organizational skills facilitate collaboration with multidisciplinary teams and enhance the overall care experience for residents.
