Salary Range: $61,000.00 to $92,000.00 Annually

Visiting Nurse Health System (Visiting Nurse) is seeking a Licensed Practical Nurse (LPN) to join our Home Health team who is a skilled nurse and a calm, compassionate, and empathetic presence caring for our patients. . The position is non-exempt and typically works Monday through Friday across Ellenwood & Lithonia. This role is headquartered at our corporate office at Ravina in Dunwoody, GA.

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provide nursing services as ordered by the physician and under the direction of the RN.
• Report subjective and objective patient findings to the RN.
• Under the direction of the RN, obtain physician’s orders and ensure orders exist for each visit made.
• Provide necessary skills to report changes appropriately; document pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrate knowledge of infection control standards and techniques per VNHS policy and procedures and consistently practice these techniques.

• LPN graduate from an accredited school of nursing or program
• Completion of an IV training program or certification in IV therapy
• Experience with advanced wound therapies
• Current license by the Georgia State Board of Nursing
• Minimum of one to two (1-2) years’ experience in a Medicare-certified home health agency is preferred
• Minimum of two (2) years as an LPN
• Current valid driver’s license, proof of auto insurance, and a reliable vehicle

Salary Range: $61,634.56-$92,451.84
