Job Title:

Physical Therapist

Location:

McHenry, IL (45 minutes NW of Chicago)

Salary Range: $38/hr to $50/hr

Work Schedule:

35 hours per week, Monday-Friday (No weekend rotation)

Job Description:

As a Physical Therapist at our facility, you will be responsible for evaluating, planning, and implementing individualized treatment programs for our residents. You will work closely with our interdisciplinary team to help our residents achieve their maximum potential in functional abilities and independence. Your primary goal will be to improve the quality of life for our residents through evidence-based therapy techniques and interventions.

Key Responsibilities:

- Conduct initial evaluations and assessments of residents' physical abilities and limitations

- Develop individualized treatment plans based on evaluation results and resident goals

- Implement treatment plans and monitor progress of residents

- Provide hands-on therapy and exercises to improve strength, balance, coordination, and mobility

- Educate residents and their families on therapeutic exercises and techniques to maintain progress

- Collaborate with other healthcare professionals to ensure comprehensive care for residents

- Maintain accurate and timely documentation of resident progress and treatment plans

- Stay current with new developments and techniques in the field of physical therapy

Qualifications:

- Bachelor's or Master's degree in Physical Therapy from an accredited program

- Current state license to practice as a Physical Therapist

- Minimum of 2 years of experience in a similar setting preferred

- Strong knowledge of evidence-based therapy techniques and interventions

- Excellent communication and interpersonal skills

- Ability to work effectively in a team environment

- Compassionate and patient-centered approach to care

- Basic computer skills and proficiency in electronic medical records
