Clinical Coordinator
Supports the mission, core values and customer service of Dialysis Care Center, LLC (DCC). Adheres to the DCC Policies and Procedures and Compliance Program including all state and federal regulatory requirements. The Clinical Coordinator is responsible for assisting with management and operation of the clinic. This position assumes full responsibility for the dialysis clinic in the absence of the Clinical Nurse Manager (CNM).
Works with Clinical Nurse Manager (CNM) on developing optimal employee staffing and patient schedules. Ensure coverage during employee absences. Coordinate Charge Nurse duties with Staff RNs acting in the relief charge capacity. Creates, maintains, and communicates efficient and timely employee schedules according to the needs of the facility. Implements and ensures compliance with all organization standards, guidelines, rules, regulations, policies, and procedures.
Ensure patient and treatment charge information and other related data are accurate and submitted on time. Monitors overtime, attendance, and PTO (approves or declines as appropriate). Utilizes DCC Labor Logic to optimize staffing, labor productivity, scheduling efficiency, overtime management, and patient treatment scheduling while maintaining safe staffing standards. Supervises clinical staff, mentors the Patient Care Technicians (PCTs), and directs the day-to-day patient care.
Assists the CNM with Corrective Action Plan development, implementation, and follow-up as required. With CNM, conduct staff meetings at least monthly or as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care. Meet routinely with the CNM to discuss personnel and patient care status, issues, and information. Serves as a resource/subject matter expert for patient needs and issues, staff education and inservice sessions as necessary, working closely with the educators, outcome managers, Anemia managers, and DCC Dashboard specialist.
Monitors and documents on the DCC Dashboards. Participation and reports off in care conferences and in QAPI. Reviews new staff's documentation for accuracy. Supervises and performs direct patient care, including but not limited to taking and monitoring vital signs, pre-treatment assessment, initiation, monitoring, termination, access hemostasis, and post-treatment assessment of dialysis. Responsible for documentation, physical nursing assessment of patients, administering dialysis medications, transcribing physician orders, assessing dialysis adequacy, drive catheter reduction initiatives.
Monitors laboratory orders, reviews laboratory results through Ascend, ensures appropriate specimen collection, packaging, shipping, and physician notification. Reports alert/panic and abnormal lab results to physician. Communicates all emergencies, patient life-threatening complications, and related situations to the attention of the medical director, physician, and Clinic Nurse Manager. Maintains competency with all emergency operational procedures. Ensures verification and availability of adequate emergency equipment.
Assesses patients' response to hemodialysis treatment therapy, making appropriate adjustments and modifications to the treatment plan as prescribed by physician, assessing target weights. Ensures compliance with safety and infection preventative measures, performing safety and infection audits on a routine basis. Ensuring patient safety and monitoring adverse Event Occurrences. Educates patients and Family members on all aspects of End Stage Renal Disease.
Collaborates with other disciplines to ensure patient outcomes are being met, instructs patients on pre and post dialysis care. Provides ongoing continuous education to patients regarding their renal disease, vascular access and dialysis therapy, fluid management, dietary modifications, medication compliance, discuss modality awareness and transplant awareness. Complete Nurse's Technical Training Program/Water Quality Facility Training. Train new and current staff as needed.
Assists with orientation of new employees. Participates in interviewing candidates. Assists with annual performance evaluations. Float between facilities as needed and work a flexible schedule (including mornings, evenings, weekends, and holidays). Ensures mandatory in-services and annual competencies are completed as required. Participates in all required staff meetings. Travel to other locations as needed. Other duties, as assigned.
Responsible for supporting the values and customer service standards of DCC. Accountable for outstanding customer service to all staff, clients and vendors. Develops and maintains effective relationships through effective and timely communication. Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.
Graduate of an accredited School of Nursing, RN or BSN. Current state licensure as a RN. Current CPR Certification.…
