The Social Worker/ Integrated Student Support Specialist plays a key role in supporting the academic, behavioral, and social-emotional success of students through school-based interventions and home-school-community partnerships. This position provides direct services to students through individual check-ins, group counseling, home visits, and case management. Additionally, the specialist coordinates services for students with 504 Plans, facilitates communication between families and schools, and connects students to internal and external resources to ensure equitable access to support.
Key Responsibilities:
Student Services & Support Provide regular individual check-ins for students requiring additional social, emotional, or behavioral support. Design, implement, and facilitate counseling groups (e.g., social skills, anger management, trauma response, self-regulation, attendance, grief). Collaborate with school-based teams to identify students in need of targeted or intensive interventions. 504 Plan Case Management Serve as the case manager for students with Section 504 Plans.
Coordinate evaluation meetings, accommodation planning, annual reviews, and plan implementation in alignment with federal and state law. Provide consultation to teachers and staff to ensure effective delivery of accommodations. Family & Community Engagement Conduct home visits to support student attendance, family communication, crisis response, and wraparound planning. Engage families in collaborative problem-solving to reduce barriers to student success and promote positive school connections.
Serve as a liaison to external agencies and community-based resources for mental health, housing, medical, and other supports. Multi-Tiered Systems of Support (MTSS) Participation Collaborate in Tier 2 and Tier 3 MTSS processes, including behavior, attendance, and emotional wellness teams. Assist in developing and monitoring attendance improvement plans and behavioral support plans. Document intervention implementation and monitor student response to supports.
Compliance & Documentation Maintain accurate, confidential records of services provided, including student check-ins, group participation, 504 meetings, and home visits. Track and report service data and outcomes for program evaluation and planning. Ensure compliance with district, state, and federal regulations related to attendance, 504 accommodations, mandated reporting, and student privacy. Professional Development & School Capacity Building Support school staff through coaching or professional learning on trauma-informed practices, restorative supports, student engagement, and cultural responsiveness.
Contribute to initiatives focused on improving school climate and student wellness.
Qualifications:
Required:
Master’s degree in Social Work, Counseling, School Psychology, or related field PA Educational Specialist certification or valid PA licensure (LSW, LPC, LSW, LCSW, or equivalent) Demonstrated experience with student case management, counseling services, and 504 Plan coordination Valid PA driver’s license and reliable transportation Knowledge of relevant Pennsylvania laws regarding 504 Plans, school attendance, and child welfare Preferred:
Bilingual proficiency Experience in public K–12 educational settings Familiarity with MTSS frameworks, trauma-informed care, and culturally sustaining practices All Grades
