Duties and Responsibilities:

Service Coordination:

  • Assess client needs through interviews and ongoing communication.
  • Develop and implement individualized service plans to address the specific needs of each client.
  • Coordinate services such as housing, education, healthcare, employment, and social services to support client well-being.
  • Work closely with community agencies, healthcare providers, and other organizations to ensure clients receive appropriate resources.

Bi-Lingual Support:

  • Provide translation and interpretation services to help non-English-speaking clients understand and navigate systems, programs, and services.
  • Translate written materials (e.g., forms, program information) from English into Spanish and vice versa.
  • Ensure clear communication between service providers and clients by serving as a language liaison during meetings and appointments.

Advocacy:

  • Advocate on behalf of clients within various systems (education, social services, healthcare, legal, etc.) to ensure their needs are met.
  • Assist clients in overcoming barriers to accessing services, including language, transportation, and understanding of the system.
  • Empower clients by providing information and guidance on how to access services independently.

Case Management & Follow-up:

  • Monitor the progress of service plans, providing support and making adjustments as needed.
  • Conduct regular follow-up with clients to ensure services are effectively meeting their needs.
  • Maintain accurate records of client interactions, services provided, and outcomes.

Collaboration:

  • Build relationships with community organizations, government agencies, schools, and healthcare providers to enhance service delivery.
  • Collaborate with internal and external teams to provide comprehensive support for clients and their families.
  • Attend meetings, workshops, and training sessions to remain informed about community resources and best practices.

Documentation & Reporting:

  • Maintain detailed, confidential records for each client, ensuring compliance with all applicable regulations and guidelines.
  • Prepare and submit timely reports on service delivery, client progress, and outcomes.
  • Utilize case management systems to track client information and services.

Community Outreach:

  • Conduct outreach to raise awareness of available services and programs, particularly within the non-English-speaking communities.
  • Engage with community members to identify individuals and families in need of support.
  • Participate in community events and information sessions to promote services and build trust within the community.

Qualifications:

Education and Experience:

  • Bachelor's degree or Associates Degree in Social Work, Human Services, Public Health, or a related field. Equivalent work experience may be considered.
  • At least 2 years of experience in service coordination, case management, or a similar role, preferably working with diverse and multi-lingual populations.

Language

Skills:

  • Fluency in English and Spanish is required. Must be proficient in speaking, reading, and writing both languages.

Knowledge, Skills, and Abilities:

  • Strong understanding of local, state, and federal resources available to families and individuals.
  • Excellent interpersonal and communication skills, particularly in working with non-English-speaking clients.
  • Strong organizational skills with the ability to manage multiple cases and tasks simultaneously.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Cultural sensitivity and understanding of issues facing diverse populations.

Technical

Skills:

  • Proficient in Microsoft Office Suite and experience using case management software is preferred.

Additional Requirements:

  • Current Act 114, Act 34, Act 151 and NSOR Clearances required prior to an offer (Must be dated within 1 year of hire date).
  • School Personnel Health Record (Form H511.340) must be completed and received by Human Resources prior to hire date.
  • Valid driver's license and reliable transportation for home visits and community meetings.
  • Ability to work flexible hours, including some evenings or weekends, to meet the needs of clients.
