Overview
As a RN Case Manager, your voice to influence patient care is valued and empowered at every turn—whether through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You'll help shape decisions that elevate both patient outcomes and the future of nursing.
Responsibilities- This is a FT Day shift position, 8:00am – 4:30pm Monday – Friday with some weekends*
- Promotes patient-centered care by coordinating the plan of care for the patient stay, managing length of stay, ensuring appropriate resource management, and developing a safe discharge plan in collaboration with the multidisciplinary team.
- Facilitates progression and transition of care using established criteria and in conjunction with the multidisciplinary team.
- Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
- Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family and assesses available resources.
- Reassesses the patient's clinical condition; considers readmission risk and develops strategies to mitigate including education on healthcare resources, preventative education, and community resources.
- Collaborates with the multidisciplinary health care team and the patient's physician to facilitate a successful care transition and discharge plan.
- Ensures post-acute medical needs and level of care are appropriate in partnership with Social Services.
- Refers to Social Services when risk factors for psychosocial determinants of health are identified.
- Engages patient and family in identifying needs and setting mutual, realistic goals.
- Evaluates progression of care using evidence-based tools and approved criteria (Inter Qual); escalates issues through the established chain of command.
- Facilitates referrals to third-party payers, disease management programs, and case management for recurring patients and those with chronic diseases.
- Supports patient throughput with focus on effective care transitions, quality and efficiency.
- Documents professional recommendations, discharge plans, care coordination interventions, and case management activities for the health care team.
- Aligns patient needs with available resources to ensure safe discharge and transition.
- Acts as a liaison through professional communication with physicians, patients/families, hospital staff, and outside agencies.
- Identifies and provides for the needs of under-resourced patients, including education, patient assistance programs, and community resources.
- Demonstrates knowledge of regulatory requirements, ethics and compliance policies, and quality initiatives.
- Advocates for patients' rights, needs, and values; ensures that ethnic, cultural, or religious values are considered and respected.
- Registered Nurse with current TX state license required
- Associate Degree in Nursing or Nursing diploma required;
Bachelor's Degree in Nursing preferred - 3+ years of clinical hospital nursing experience required OR 2+ years of case management experience
- Certification in case management preferred
- Inter Qual experience preferred
