Key Responsibilities Include- Providing comprehensive home health nursing care to patients
- Assessing patient needs and creating individualized care plans
- Monitoring patient progress and adjusting treatments accordingly
- Collaborating with healthcare teams to ensure quality care
- Documenting patient interactions and maintaining accurate records
- Minimum of 1 year full-time RN Home Health experience within the last 2 years
- Current RN license
