Wheeler Staffing Partners seeks an experienced RN Patient Care Manager for direct hire in McKinney, TX. The role oversees daily operations of a home health team, ensuring high‑quality, patient‑centered care through clinical leadership, staff supervision, regulatory compliance, and partnership with patients, families, and providers.
Key Responsibilities- Clinical Leadership: Supervise and coordinate delivery of home health services; lead, mentor, coach, and support multidisciplinary team; develop, review, and oversee individualized patient care plans; monitor patient progress and adjust plans; serve as clinical resource promoting evidence‑based practices and quality standards.
- Operations & Compliance: Ensure compliance with federal, state, local regulations, Medicare Conditions of Participation, accreditation standards, and company policies; monitor quality metrics, patient outcomes, and performance indicators; conduct staff meetings, coaching sessions, and clinical education; collaborate with physicians, referral sources, patients, caregivers, interdisciplinary teams; support recruiting, onboarding, performance management, employee engagement, and staff development.
- Administrative Responsibilities: Oversee scheduling, staffing, productivity, documentation, and daily operational workflows; assist with billing, reporting, and operational performance initiatives; maintain accurate clinical and operational documentation; identify opportunities to improve efficiency, regulatory compliance, quality, and patient satisfaction.
- Active Registered Nurse (RN) license in the State of Texas.
- Minimum of two (2) years of Home Health experience.
- Minimum of two (2) years of Home Health supervisory or management experience.
- Strong knowledge of home health regulations, clinical operations, and patient care standards.
- Proven leadership experience managing and developing clinical teams.
- Excellent organizational, critical thinking, and decision‑making skills.
- Strong verbal and written communication skills.
- Ability to manage multiple priorities in a fast‑paced healthcare environment.
- Proficiency with Microsoft Office and electronic medical record (EMR) systems.
- Bachelor’s degree in Nursing, Healthcare Administration, or a related field.
- Experience leading quality improvement initiatives within a home health organization.
- Knowledge of Medicare home health reimbursement and regulatory requirements.
- Experience with home health EMR platforms.
