Home Health Patient Care Manager

Responsible for supervising and coordinating clinical services within a home health setting, ensuring high-quality patient care in compliance with all relevant laws and regulations. Leads an interdisciplinary team, manages patient assessments, plans, and care delivery, and verifies insurance coverage and benefits.

• Provides clinical services within the scope of nursing practice, aligned with physician plans of care
• Receives referrals, assigns clinicians, evaluates patient orders, and initiates start of care visits
• Coordinates patient benefits, medical necessity, and insurance approvals
• Assesses patient needs continuously, ensuring individualized, appropriate care that meets eligibility criteria and physician directives
• Reviews assessments and care plans daily, offering clinical recommendations
• Operates effectively in diverse home environments, respecting all client backgrounds

• Current unrestricted RN licensure in the state of practice
• Valid driver’s license, vehicle insurance, and access to reliable transportation

• CPR certification or ability to obtain within 90 days
• Home care experience

This role offers recognition and development opportunities, with a salary range of $89,680 to $134,520 annually, supplemented by comprehensive benefits and incentive programs.
