Description

This position's responsibilities include in-person, virtual, telephone and community-based services such as individual and group sessions with the individual, their family members and/or significant others to address recovery issues and help keep them engaged in their recovery. The Recovery Coach will also assist with transportation to referral services or recovery meetings. The Recovery Coach will assist in identifying other barriers to the individual's recovery and provide resources.

Some examples include housing, food insecurity, employment, mental health, legal issues.

This position exemplifies service excellence by providing services with a trauma-informed and person-centered approach. The Recovery Coach will provide flexible hours of service based on the needs of individuals seeking support services. The Recovery Coach will document all services, outreach, engagement and follow-ups via EHR and CMBHS system as outlined by HHSC. Documentation will be completed in a timely manner and accurately to reflect services being provided.

The Recovery Coach will track and document financial assistance provided to individuals and assist in reporting the required measures as outlined in the contract, if applicable.

This position practices good stewardship of resources by good utilization of workforce time, utilization of agency resources, leveraging of internal/external resources to meet the needs of the individual and creating value as a professional in knowledge, skills and abilities to perform these duties. Integrity is necessary to ensure adherence to the Texas Administrative Code, contracts, other applicable grants, and Lifepath Systems policies and procedures.

This position will work closely with the Program Administrator for SUD services and Peer Support Supervisor to ensure adherence to contract requirements is being met.

The Recovery Coach will build positive relationships with community-facing agencies and always maintain professional relationships with all community stakeholders and partners. This position demonstrates professional conduct with providers, peers, leadership and other support teams at all times.

The Recovery Coach will be expected to work independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals being served. This position through service delivery will assist in enhancing quality of life, resiliency and increased social connections through sustained engagement in long term recovery support services. This project will show improvement in measurable ways, which is central to Life Path Systems' mission to serve as well as the purpose of this project.

Specific Targets:
The Recovery Coach will provide in person, telehealth or telephonic contact daily for those individuals enrolled in recovery support services. The Recovery Coach will facilitate individual sessions, recovery support groups and community-based services to individuals on their caseload. Frequency of sessions will be based on individual need, and the Recovery Coach will be required to complete an assessment and recovery plan at the time of enrollment to identify the needs of the individual.

The Recovery Coach will establish effective linkages with the OSAR team, substance use treatment providers, Texas Workforce Commission, other recovery support organizations, community-based organizations, and other sources of support within the Community by doing outreach and engagement with these entities. The Recovery Coach will attend community events to ensure information about recovery support services is available to community entities and to build those community partnerships.

The Recovery Coach will enroll individuals in long-term coaching when the individual shows a need and willingness to participate in recovery support on a long-term basis.
