Position: LPN Charge Nurse Home Health- PRN (As Needed)

* Graduate of an accredited nursing program

* Current LPN license in the Commonwealth of Virginia

* Current CPR certification

* One year of experience in medical/surgical, long-term care, or Assisted Living preferred

* Experience working with older adults preferred

Responsibilities

* Provide direct care to residents

* Assist with resident assessments and report any changes in condition

* Communicate with physicians to update care plans when needed

* Administer medications and treatments and document all care

* Help coordinate pharmacy services for residents

* Supervise nursing staff when serving as Charge Nurse

* Monitor residents and ensure proper documentation is completed

* Conduct rounds to ensure care and policies are being followed

* Respond to emergency situations and document appropriately
