Moms In Motion / At Home Your Way is seeking a Service Facilitator (SF) Case Manager who must live in VA. You’ll hit the road locally to visit client homes and help families navigate Virginia’s Medicaid Waiver programs to promote safe, healthy, and independent lives.
No degree is required; 2+ years of experience with disabilities or elderly is preferred. You’ll write care plans, resolve authorizations and timesheets, and build trusted, lasting relationships with families.
