Licensed Practical Nurse

Life Care Centers is seeking an LPN Licensed Practical Nurse to provide compassionate, high-quality care to our senior residents. In this role, you will deliver direct patient care, administer medications, monitor vital signs, and support rehabilitation plans under RN and physician direction. You'll collaborate with an interdisciplinary team to promote safety, comfort, and optimal outcomes, while educating residents and families. Responsibilities

• Provide direct nursing care to senior residents in accordance with care plans and facility policies
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, patient status, and responses to treatment
• Assist with rehabilitation and restorative care activities
• Collaborate with RNs, physicians, therapists, and caregivers to coordinate care
• Maintain accurate, timely documentation in electronic health records
• Educate residents and families on care plans, medications, and wellness
• Promote a safe, clean, and compassionate environment for residents
• Recognize and report changes in resident condition promptly
• Support quality improvement and regulatory compliance initiatives
