All LCSNW team members will contribute to our mission of partnering with individuals, families and communities for health,justice and hope. This individual will provide services to families self-referred and through DHS/Self Sufficiency to enhance family stability and child safety. The individual will work with families in their homes and the community to conductcomprehensivescreening todetermineimmediate needs, short- and long-term goals, and unique family barriers to create a collaborative goal/service plan.

Provides skills training to children,adolescents and adults to address immediate needs and implements a plan forward to overcome barriers in accessing resources to meet those needs. Creates a strong rapport with professional boundaries and provides connection by referral to outside resources needed to further support the family in reaching their goals.

Collaborates with community partners and connects families to relevant resources to support progress.

  • Provide in-homeservices to children,adolescents and families
  • Assessment of parental capacities related to child safety and well-being, basic needunmetand extraordinary barriers preventing family stability
  • Help family members overcome barriers andidentifystrengths
  • Report any potentially or dangerous situations to immediate supervisor immediately
  • Use Motivational Interviewing, Collaborative Problem Solving, Wraparound, and other effective practices
  • Knowledge of dynamics of high-risk families including substance use and behavioral health
  • Coordinate case activities with Contract Supervisor, DHS/Self-Sufficiency,schools and other community partners
  • Provide a range of services including parent education, modeling healthy communication skills, empowering parents to self-advocate, locating appropriate resources to meet basic and complex family needs,assistin barriers to home management, budgeting, stress reduction, stabilization of home environment, and other appropriate services as needed
  • Maintain accurate and current client records documenting ALL client contact including weekly progress notes, short- and long-term goals, monthly progress reports, collaborative staffing reports with other SSP teammates, and completing initial screening and releases uploading to client profile within the electronic client record (ECR); sign documents to meet contract requirements.
  • Refer client to supportive services to supplement treatment and counseling as appropriate
  • Conduct parent-child coaching and interaction sessions
