Responsibilities

* Assess and evaluate patient needs to develop and implement individualized care plans.

* Coordinate and oversee the delivery of patient care in a home setting.

* Serve as a primary contact for patients and their families regarding care and treatment.

* Collaborate with healthcare teams to ensure comprehensive patient care.

* Monitor and document patient progress and outcomes in compliance with policies.

* Conduct patient education regarding their health, care plans, and disease management.

* Manage clinical documentation and ensure adherence to regulatory standards.
