Position: FAMILY COUNSELOR II, STEPPING STONES-FULL TIME
JOB DUTIES

Maintains communication with other members of the chemical dependency treatment team.

Acts as Case Manager for patients, functioning as a social worker when necessary to prepare patients for discharge.

Makes appropriate, legible, entries in patient record

Counsels patients on an individual basis.

Conducts group therapy as primary facilitator.

Prepares Master Treatment Plans, Psychosocial interview forms and meaningful Aftercare Plans in concert with the needs of the patient.

Works variable hours as directed by the Program Supervisor to meet department and patient needs.

Conducts family education sessions and works on an individual basis with families when so directed.

Functions as a member of the Treatment Team.

Possess knowledge of and utilizes referral sources and other agencies necessary to provide therapeutic support for the patient.

Collaborates with other hospital departments in order to maximize the effect of the current hospital admission.

Operates all audio-visual equipment, facsimile machine, and photocopier.

Implements patient activities in cooperation with the Occupational Therapy Department.

Assesses level of care per Pennsylvania Client Placement Criteria (PCPC). Completes Criteria Checklist for all clients to verify and substantiate the appropriate level of care.

Understands and assesses DSM-V diagnostic codes and axes.

AGE/DEPARTMENT SPECIFIC COMPETENCIES

Demonstrates the ability to perform age specific assessments in the adult and geriatric population.

Uses appropriate communication skills for specific ages in the adult and geriatric population.

Assesses safety issues specific to age in the adult and geriatric population.

MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

Bachelor's Degree in human services or counseling is required. Master's Degree preferred.

Must be licensed in Pennsylvania as a Professional Counselor (L.P.C.,) Licensed Social Worker (L.S.W.,) Licensed Clinical Social Worker (L.C.S.W.,) or Licensed Marriage and Family Therapist (L.M.F.T.).

Must be eligible to pursue a Certified Alcohol and Drug Counselor (C.A.D.C.) in Pennsylvania or other approved counseling related certification.

A minimum of two (2) years' experience as a drug and alcohol counselor.

Must have knowledge and understanding of the 12-Step Recovery Program.

Must have attained one or more National or Pennsylvania approved counseling certifications such as Certified Alcohol and Drug Counselor (C.A.D.C.), National Certified Counselor (N.C.C.), Certified Clinical Mental Health Counselor (C.C.M.H.C.) or Certified Rehabilitation Counselor (C.R.C.) or be able to achieve certification with acquired hours of employment.

Communication skills necessary to effectively communicate with other members of the multidisciplinary/health team members, referral sources, A.A., N.A., Social Service Agencies, ACOA, Al-Anon, patients and their significant others.

Must be able to understand the role of a counselor as it relates to the ability to differentiate between enabling and supporting, and then utilize these skills in counseling.

Possess effective interpersonal communication, interviewing skills, and the ability to establish therapeutic relationships.

Must be able to have the medical knowledge to explain and defend the disease concept of addictions and all the medical ramifications.

Must be certified in Pennsylvania Client Placement Criteria.

Knowledge of DSM-V diagnostic codes and axes.

Documentation must ensure appropriateness for the intensity of service in a timely and efficient manner.

Ability to make decisions and exercise sound judgment in the interest of the welfare of the patient.

WORKING CONDITIONS

Normal patient care environment with little exposure to excessive noise, dust, and temperature changes.

A large part of work time is sitting, standing or walking.

Stress is a large factor in the environment.

Must be able to set priorities and handle large volumes of work.

Subject to certain distasteful patient care activities and exposure to communicable diseases.

Scheduled hours of work may include weekends and/or holidays.

May be exposed to potential hazards such as chemical wastes, body wastes, fluids and other possible infectious material.
