What You'll Do
As a Hospice RN - Regional Support, you'll provide direct patient care while serving as a floating clinician assigned to hospice branches based on census needs, growth opportunities, or staffing support.
Responsibilities include:
+ Managing and coordinating hospice patient care
+ Performing comprehensive patient assessments
+ Developing and implementing individualized care plans
+ Educating and supporting patients, families, and caregivers
+ Collaborating with interdisciplinary hospice teams
+ Providing compassionate care that promotes comfort, dignity, and quality of life

As a Regional Support Hospice RN, You Will:
* Deliver high-quality, compassionate nursing care for hospice patients
* Manage and implement individualized plans of care
* Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
* Assess and address patients' physical, emotional, spiritual, and environmental needs
* Accurately document all care provided and ensure compliance with hospice protocols
* Educate families and caregivers on care instructions, medications, and safety
* Participate in the on-call rotation and provide after-hours care as needed
* Collaborate with physicians, referral sources, and the care team
* Assist with referrals, regulatory documentation, and payer compliance
* Travel to various Gentiva hospice locations as assigned to fill urgent needs
Schedule & Travel:
This role typically follows a Monday-Friday schedule, with occasional on-call responsibilities. Travel is required and may include:
+ Regional travel with overnight stays based on assignment needs.
_A company fleet car is provided for eligible employees._
About You
Qualifications - What You'll Bring:
* Current, unencumbered RN license in the state of employment
* Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
* Current CPR certification
* Valid driver's license, auto insurance, and reliable transportation
Preferred Experience (Not required):
* Hospice and end-of-life nursing experience
* Skilled in clinical assessments and compassionate communication
* Strong critical thinking, flexibility, and problem-solving skills
* Comfortable working in varied environments with shifting needs
* Proficient in documentation and electronic health records
* Calm and supportive in crisis situations; skilled at family engagement
