GENERAL OVERVIEWThe Registered Nurse assesses human responses and plans, implements and evaluates nursing care for individuals or families for whom the nurse is responsible. The Registered Nurse is fully responsible for all actions as a licensed nurse and is accountable to patients for the quality of care delivered.
ESSENTIAL RESPONSIBILITIES- Assures safe delivery of nursing services in alternative care settings.
- Active participation in ongoing interdisciplinary communication for each patient, reviewing patient status and plan of care.
- Directs and organizes ancillary care as appropriate.
- Adheres to Allegheny Health Network policies and procedures regarding medical record documentation and Performance Improvement activity.
- Administers infusion therapy during Home Infusion visits in the home setting.
- Implements current nursing practices.
- Completes patients’ clinical assessments as appropriate and communicates them to other team members.
- Implements nursing interventions to meet patient’s needs in accordance with physician orders.
- Receives, implements, and evaluates physician orders for patient outcome in a timely manner.
- Refers patients to other health care support systems or community resources based on identified needs.
- Documents referral information accurately and shares it with other team members in a timely manner.
- Implements and monitors skilled nursing care per the Home Infusion policies and procedures.
- Communicates patients’ status and ongoing needs at discharge to appropriate individuals.
- Assumes responsibility for nursing services on weekends, holidays, and after‑hours on‑call as assigned.
- Serves as Home Infusion on‑call for emergencies and after‑hours visits as necessary.
- Promotes effective written and verbal communication.
- Promotes a positive work environment for other staff members.
- Demonstrates timely communication regarding changes in plans of care to other team members.
- Makes sure lines of authority are consistent and appropriate.
- Participates in special committee meetings as required.
- Cooperates with other health care team members and the Operations Manager pursuant to the organization’s philosophies and objectives.
- Gives accurate information to patients, caregivers, and other health care providers about services and associated costs.
- Demonstrates knowledge of policies and procedures and regulations regarding reimbursement for nursing and pharmacy services.
- Consults with the Patient Account Representative regarding reimbursement status of patients.
- Uses equipment and supplies effectively and efficiently.
- Provides teaching and care to patients/caregivers for pharmacy services patients.
- Maintains direct physician contact (e.g., patient care communications, marketing).
- Coordinates medication and supply deliveries to patients.
- Provides information regarding drugs and their safe and appropriate use to other health care providers involved in patient’s or client’s care.
- Actively participates in ongoing interdisciplinary communication specific to each patient.
- Participates in the Performance Improvement program.
- Participates in in‑service programs for other health care professionals and for the community.
- Attends seminars/CEU opportunities on an ongoing basis to promote growth and effective performance.
- Possesses thorough knowledge of and strict adherence to the organization’s Mission, Vision, and Core Values.
- Performs other tasks as assigned.
- Graduate of an accredited School of Nursing.
- Licensed and in good standing with the State Board of Nursing in state of practice.
- One year of acute care/IV experience.
- Act 34 Criminal Background Clearance Certificate.
- Act 33 Child Abuse Clearance Certificate.
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
- Current State of PA RN licensure or current multi‑state licensure through the enhanced Nurse Licensure Compact (eNLC).
- CPR certification.
- Current valid driver’s license.
- Proof of current automobile insurance.
- Experience with Home Infusion Care and/or IV Therapy.

COMPENSATIONPay Range Minimum: $32.00
Pay Range Maximum: $50.75
