This assignment is ideal for an RN experienced in discharge planning, resource coordination, patient advocacy, and guiding patients through post‑acute care transitions.

• Conduct detailed discharge and resource assessments
• Develop individualized care plans in collaboration with providers and therapy
• Facilitate transitions to SNF, rehab, home health, and outpatient services
• Perform utilization reviews and insurance coordination
• Educate patients and families on next steps, instructions, and community resources
• Maintain accurate EMR documentation
• Serve as a patient advocate ensuring safe and efficient transitions

• Active North Carolina RN license or compact eligibility
• Current BLS certification
• Minimum 1–2 years of recent Case Management experience
• Strong communication, assessment, and critical‑thinking skills
