This assignment is ideal for an RN experienced in discharge planning, resource coordination, patient advocacy, and guiding patients through post‑acute care transitions.

- Active North Carolina RN license or compact eligibility
- Current BLS certification
- Minimum 1–2 years of recent Case Management experience
