Triage RN, Family Medicine

We are seeking a Full‑time 40‑hour RN to support our practice onsite. This position will work Monday‑Friday from 8:30 am–5:00 pm, with the potential to become hybrid in the future.

The RN will work collaboratively with physicians, advanced practitioners, and other healthcare professionals to deliver patient‑centered care, focusing on health promotion, disease prevention, and the management of acute and chronic conditions.

This role is primarily phone triage with patients and some patient‑facing duties. The ideal candidate is an adaptable RN who is confident in clinical abilities and enjoys variety.

• Triage clinical questions via phone calls.
• Provide clinical advice and education to patients based on assessment findings and established protocols.
• Provide appropriate care coordination for patients such as scheduling appointments, follow‑up care and referrals to specialists and other healthcare services.
• Follow‑up on emergency room visits.
• Collaborate with physicians, advanced practitioners, and other healthcare team members to ensure comprehensive patient care.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Provide direct patient care, including assessments, planning, implementation, and evaluation of care plans.
• Administer injections, medications, treatments, and procedures as prescribed by physicians and advanced practitioners and assist in minor surgical procedures.

Associate's Degree Nursing required;
Bachelor's Degree Nursing preferred.

Registered Nurse [RN – State License] – Generic - HR Only required.

Basic Life Support [BLS Certification] – Various issuers preferred.

Clinical nursing experience 0‑1 year required;
Ambulatory care or outpatient experience 1‑2 years preferred.
