Physical Therapist Assistant (PTA) - $10,000 Sign-On Bonus! Hearthstone Nursing & Rehabilitation Center | 2901 E Barnett Rd, Medford, OR 97504. Pay: $33/HR – $42/HR, Depending on Experience.

• Implement individualized treatment plans developed by the Physical Therapist
• Collaborate with nursing and facility staff to support resident goals
• Track progress and ensure quality outcomes
• Maintain strong documentation and communication

• Licensed as a Physical Therapist Assistant (PTA) in Oregon or graduate of an approved PTA program or actively working toward licensure
• Strong communication and collaboration skills
