Registered Nurse

Job Summary:

The Registered Nurse will provide skilled nursing care to patients in their homes within the Medford, OR, and surrounding areas. This role involves patient visits, adherence to productivity standards, and collaboration with the healthcare team. The RN must have their own transportation and be willing to travel within a radius that can occasionally extend to 50+ miles.

Responsibilities:

- Provide skilled nursing care to patients in their homes.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Document patient care accurately and timely using EPIC/Oasis charting system.
- Maintain productivity standards, potentially visiting up to 6 patients per day (30 productivity points standard).
- Communicate effectively with patients, providers, and team members.

Required Skills &

Certifications:

- Active Registered Nurse license in the state of working.
- BCLS certification.
- Minimum of 1 year of skilled home health experience.
- Oasis experience.
- Driver's license, car insurance, and reliable transportation.
- Primary series COVID vaccination in addition to one booster.

Special Considerations:

- Traveler is expected to have their own transportation.
- Occasional travel radius of 50+ miles may be required.
- Facility can call off UP TO one shift per pay period (every two weeks).
- Local candidates within a 50-75 mile radius are accepted at the same rate.

Scheduling:
- 8-hour day shifts.
- The facility cannot accommodate 4x10s or 3x12s schedules.
- No weekend requirement.
- No on-call requirement.
- Block scheduling is not accommodated.
