Travel Physical Therapist - Home Health

American Traveler is seeking an experienced Physical Therapist for a Home Health position in Medford, OR and surrounding areas, requiring at least 1 year of skilled home health experience and Epic/OASIS proficiency.

Job Details

• Home health setting serving Medford, OR and surrounding areas,

• 8-hour day shifts, 5 days per week (4x10 and 3x12 schedules are not available),

• Patient visits up to 6 patients per day with a standard productivity expectation of 30 points,

• 1:1 patient care model,

• Charting via Epic and OASIS,

• No weekend or on-call requirements,

• Travelers are expected to float between all facilities within the state,

Job Requirements

• Minimum 1 year of skilled home health PT experience required,

• BLS certification required,

• Epic EMR experience required,

• OASIS experience required,

• Reliable personal transportation required,

• COVID-19 primary vaccination series plus at least one booster required,

• License required if applicable,

• Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting,

Additional Information

• First-time travelers will not be considered for this position,

• Local candidates within a 50-75 mile radius are accepted at the same rate,

• Former permanent staff must wait at least 3 months before returning as a traveler and must reside more than 50 miles from the submitting facility,

• Candidates with a pending license will be considered,

• Any requested time off should be disclosed upfront, as post-offer RTO requests may be denied,

• The scheduling system groups patient visits to minimize drive time, though occasional travel of 50+ miles may be required,

• Any color scrubs are acceptable,
