Job Summary

Licensed Practical Nurse – Home Health (LPN) with Providence Medford Medical Center, OR.
Full time, day shift. $5,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment. Covers Josephine and Jackson counties.

• Work under the direction of a registered nurse to provide quality nursing care to patients in their homes.
• Assist with collection of assessment data and provide input to the planning of care.
• Implement plan of care within the scope of licensure.
• Perform all duties in a manner that promotes team concept and reflects the Providence mission and philosophy.

• Graduate of an accredited LPN/LVN school of nursing.
• National Provider BLS – American Heart Association within 30 days of hire.
• Oregon Licensed Practical Nurse upon hire.
• Washington Licensed Practical Nurse upon hire if practicing within this state.
• 1 year experience in nursing (long‑term care, acute care, ambulatory care, home health or hospice care).
• Criminal background check upon hire.

• Experience in home health, hospice or public health.
