0.6 FTE available; weekend work may be required.The RN Case Manager is an expert registered nurse responsible for developing and managing patient care outcomes for an entire patient case load. Duties include assessment, treatment, or care for patients of all ages—neonate, pediatric, adolescent, adult, and geriatric—based on population focus, as well as complex discharge planning and utilization review.

Required Qualifications:- Upon hire:
Oregon Registered Nurse License. - 3 years Acute Care hospital experience.
- Bachelor's Degree in Nursing.
- 1 year preferred experience in home health, mental health, substance use, hospice, and/or utilization review.
- Preferred progressive nursing leadership experience, including charge nurse role.

Posted are the minimum and maximum wage rates for this position. Your placement on the wage range will be determined by relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
