a licensed practical nurse (lpn) working in our community based care facility will work to maintain the quality of care of all residents and to support the philosophy of the community. This position requires flexibility, time management skills and the knowledge base of the geriatric population who may have complex medical needs.

• provide primary nursing care to residents with varying backgrounds and medical histories
• monitor and communicate change of condition for residents
• oversee the medication administration program
• provide medical intervention as needed for emergent needs
• a licensed practical nurse (lpn) will implement and follow primary care providers' orders
• communicate needs of resident to outside providers including but not limited to: family, primary care provider, home health agency, pharmacy, etc
• assist in supervision of the health services staff as needed
• other duties as assigned
