Schedule:
Full Time, Day Shift Areas of coverage:
Josephine and Jackson counties
Works under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assists with collection of assessment data and provides input to the planning of care. Implements plan of care within the scope of licensure. Performs all duties in a manner which promotes team concept and reflects the Providence mission and philosophy. Licensed Practical Nurses (LPNs) work alongside an interdisciplinary team to provide whole-person quality care to patients in need.

- Graduate of an accredited LPN/LVN school of nursing
- National Provider BLS - American Heart Association within 30 days of hire
- Oregon Licensed Practical Nurse upon hire
- Washington Licensed Practical Nurse upon hire - if practicing within this state
- 1 year experience in nursing; long term care, acute care, ambulatory care or home health and hospice care

Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. - In compliance with the Providence St. Joseph Health - OR, criminal background check upon hire.
- Experience in home health, hospice or public health
$27.82 - $42.56 (base pay range; additional compensation may be available such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities)
