The RoleIdeal candidates thrive working with a collaborative and compassionate community of caregivers. For this position, Nurse Practitioners will manage patients in an outpatient clinic setting as well as providing in‑home clinical support for patients in the community. GGH Nurse Practitioners will support clients with their patient panels and partner with clinics as an embedded member of the care team with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients’ medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare.
Whatwill you do as Nurse Practitioner at GGH?
- Assess patients’ clinical problems and healthcare needs and provide clinical care and consultation
- Perform an annual wellness exam for senior Medicare patients in a designated clinic or at their place of residence.
- Obtain and record the patient’s medical history and symptoms, including medicine reconciliation.
- HCC coding, verification, and risk adjustment.
- Coordinate continuity of care, prevention and avoidance of complications, and close clinical treatment and management under the direction of the patient’s primary healthcare provider(s) and collaboration with other referral sources.
- Perform various tests and screenings.
- Counsel patients with education around their discharge plan.
- Execute the discharge plan and use critical thinking skills to build an effective care plan during a critical transition period after a hospital discharge.
- Support patients and their families, and provide compassionate care.
- Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings to collaborate with other healthcare professionals (social workers, case managers, and other clinicians) to ensure the holistic approach to the care plan is carried out.
- Collaborate with care team to ensure completion of holistic patient care.
- Travel within the Medford, OR area 5 days a week to provide care to patients in their home based on the needs of business/market.
- Complete charts within 72 hours of patients’ visits; access EMR and/or portals to review charts and document notes.
- Cover for other Nurse Practitioners and Care Team Members when necessary.
- Maintain patients’ confidence by completing and safeguarding medical records, keeping patients’ information confidential, and abiding by all HIPAA rules and regulations.
This position is part‑time and may include in‑clinic, in‑home visits or telehealth visits. Home visits will be scheduled within a 30‑mile radius of your home region (Jackson County, OR).
