Position
Part Time Physician Assistant – High Risk Program (OR License Required)
Location:
Medford, OR (On‑Site)
- Assess patients' clinical problems and healthcare needs and provide clinical care and consultation
- Perform annual wellness exams for senior Medicare patients in a designated clinic or at their place of residence. Obtain and record the patient's medical history and symptoms, including medication reconciliation
- Handle HCC coding, verification, and risk adjustment
- Coordinate continuity of care, prevention and avoidance of complications, and close clinical treatment and management under the direction of the patient's primary healthcare provider(s) and collaboration with other referral sources
- Perform various tests and screenings
- Counsel patients with education around their discharge plan
- Execute the discharge plan and use critical thinking skills to build an effective care plan during a critical transition period after a hospital discharge
- Support patients and their families, and provide compassionate care
- Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings to collaborate with other healthcare professionals (social workers, case managers, and other clinicians) to ensure the holistic approach to the care plan is carried out
- Collaborate with care team to ensure completion of holistic patient care
- Travel within the Jackson County, OR area up to 5 days a week to provide home-based care to patients as needed
- Complete charts within 72 hours of patients’ visits; platform access to EMR and/or portals will be provided for chart review and documentation
- Cover for other APPs and Care Team Members when necessary
- Maintain patient confidentiality by completing and safeguarding medical records, abiding by all HIPAA rules and regulations
- Provide care in outpatient clinic, in-home visits, or telehealth visits as required; home visits are limited to a 30‑mile radius of the employee's home region (Jackson County, OR)
- Current board‑certified PA
- At least 2 years of clinical experience as an NP in primary care, family medicine, internal medicine, gerontology, or palliative care
- Acute care experience with high‑risk patients is a plus
- Home health experience (PA or RN) is a plus
- Working knowledge of chronic care management and various medication options
- Current DEA licensure / prescriptive authority post‑hire
- Active CPR/BLS certification
- Ability to execute thoroughly in a fast‑paced work environment and continually build upon best practices for efficiency
- Effective rapport building with the aging and elderly population
- Detail‑oriented with a focus on routine work
- Openness to feedback from the clinical and leadership team to strive for constant improvement with coding and processes
- Comfort with technology systems and commitment to timely EMR documentation, with deep understanding of Medicare HCC coding and managed care
- Flexible with work schedule and setting as needed
- Ability to travel up to a 35‑mile radius within Jackson County, OR 5 days a week
