School Based Pediatric/Family Nurse Practitioner

We are seeking an experienced pediatric or family nurse practitioner to work at our school-based sites 5 hours per day, Monday through Friday. This role focuses on providing primary medical and behavioral health care for children, supporting their emotional and social needs, and collaborating with primary care providers both internally and externally. The goal is to keep children in school and provide accessible primary care when barriers exist.

• Provide primary medical care to children in school-based sites.
• Support the emotional and social needs of youth.
• Collaborate with internal multi-disciplinary teams and external primary care providers to ensure comprehensive care.
• Maintain continuity of care and facilitate referrals to appropriate services.
• Keep children in school as much as possible and address barriers to accessing care.

• Graduate from an accredited school with board certification or eligibility.
• Experience with low‑income children and families and in migrant and community health preferred.
• English fluency required;
  Spanish proficiency valued.
• Active Oregon license.
• CPR certification and active DEA license.
