Nurse Practitioner Or Physician Assistant

Great opportunity within the Medford, OR area for a Nurse Practitioner or Physician Assistant to perform annual wellness exams working for a private group. Board Certified Nurse Practitioner (NP) or Physician Assistant (PA) 2+ yrs exp as NP/PA required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus Part Time or Full Time hour available - 3-5 days a week Mon-Fri 8a-5p Perform an annual wellness exam for senior Medicare patients Collaborate with Primary Care team to ensure completion of holistic patient care This role is a mix of outpatient in-clinic setting, in-home health visits, and occasional telehealth appointments Must have active OR medical and drivers license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have AWV exp.

8-10 patients per day Salary based on experience $125-150K with a sign on bonus and/or relocation assistance and plenty of other great benefits
