PRN Licensed Practical Nurse

Snap Care is seeking a PRN Licensed Practical Nurse to deliver compassionate, high-quality patient care at partner healthcare facilities in Medford, OR. In this flexible, as needed role, you'll perform routine nursing interventions, administer medications, monitor and document patient status, and support RNs and providers in treatment and care coordination. Responsibilities

• Provide direct nursing care within LPN scope, including vital signs, wound care, and basic assessments
• Administer prescribed medications and treatments safely and accurately
• Monitor, document, and report changes in patient condition to RNs and providers
• Support care plans and collaborate with multidisciplinary teams to enhance outcomes
• Maintain accurate, timely clinical documentation in facility systems
• Follow infection control, safety, and regulatory standards at all times
• Communicate clearly and compassionately with patients and families
• Adapt to varying facility protocols, units, and patient populations
