** Step into a pivotal position that combines compassion with expertise, as you facilitate Medicaid eligibility and coordinate essential services, enhancing the quality of life for individuals requiring long-term care support.
** SUMMARY OF DUTIES
** In this position you will determine initial and on-going Medicaid eligibility for long term services and supports (LTSS), to develop and implement service plans, to coordinate the provision of services for community-based care and nursing homes, and to regularly review and update Medicaid eligibility and monitor service plans.
** MINIMUM QUALIFICATIONS**
* ** A bachelor's degree in a Behavioral Science, Social Science,
** or a closely related field,
** OR**
* ** A bachelor's degree in any field AND ONE YEAR of human services related experience** (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing),
** OR**
* ** An associate degree in a Behavioral Science, Social Science
** or a closely related field
** AND TWO YEARS of human services related experience** (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing),
** OR**
* ** Three years of human services related experience** (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
** ESSENTIAL ATTRIBUTES
**** We are specifically looking for candidates with the following:**
* ** Experience using excellent communication skills to be able to discuss/assess client needs.**
* ** Experience providing excellent people/customer service skills.**
* ** Experience obtaining information through interviews.**
* ** Experience preparing and maintaining written documentation of actions taken/needed.
