in Jackson County.
The Long-Term Rent Assistance (LTRA) program provides rent subsidy to eligible households to increase
housing access and long-term housing stability.

POSITION SUMMARYThe Hearts With A Mission Long-Term Rental Assistance Case Manager (LTRA Case Manager) works as part of an inter-disciplinary service team with participating clients. This position is responsible for assessing client needs, developing individualized service plans, coordinating services and supports, and monitoring client progress toward identified goals. The LTRA Case Manager ensures that accurate and complete information is collected, documented, and maintained to support effective case management and timely delivery of appropriate services.
Through a detailed understanding of client needs, available community resources, and program requirements, along with compassionate interaction with clients, families, care teams, and community partners, this position will serve as a link between clients and the services and supports identified through their individualized service plans. Building positive, supportive relationships with clients is central to the effectiveness of this position. The LTRA Case Manager performs assignments with supervision or independently, requiring initiative and judgment in the conduct of work.
All work is performed in compliance with organizational guidelines and policies, as well as all applicable laws.
The Long-Term Rental Assistance Case Manager reports directly to the Services Coordinator. This is not a supervisory position. This position has an introductory period of three (3) months.

Client Services- Conduct screening assessments to determine community members’ eligibility for LTRA services.
- Develop individualized service plans in collaboration with clients and families to identify goals, needs, and appropriate supports.
- Provide ongoing case management and support to clients to promote long-term housing stability, engagement, and progress toward identified goals.
- Coordinate services and supports between clients, families, providers, community partners, and other members of the client’s support team.
- Maintain regular and ongoing communication with clients, families, service providers, and other appropriate supports.
- Monitor client progress and follow up to ensure service plan goals and identified needs are being addressed.
- Provide advocacy and support to assist clients in navigating systems, addressing barriers, and accessing appropriate services and resources.
- Collaborate with interdisciplinary teams and service providers to ensure coordination of services and alignment with each client’s service plan.
- Assist clients in identifying and strengthening natural supports and community connections that promote stability and independence.
- Provide information, education, and resources to clients and families to support self-advocacy and informed decision-making.
- Review client needs and service plans to identify barriers, changes in circumstances, and additional supports that may be needed.
- Support clients in establishing connections to educational, vocational, employment, housing, transportation, and other community-based opportunities consistent with their identified goals.
- Promote trust and engagement through consistent communication and supportive relationships with clients and families.
- Consult with Program Managers and appropriate team members regarding client needs, service coordination, barriers, and ongoing case management.
